What is Transform?

Transform is a digital diabetes prevention program based on a clinically-proven curriculum. It is delivered remotely by real health coaches through a smartphone app.

Program Goals

  • Significantly reduce your risk of developing type 2 diabetes

  • Lose between 5% and 7% of your starting body weight

  • Gradually increase your physical activity to at least 150 minutes each week

  • Adopt a healthier lifestyle that you can sustain for the rest of your life

The Transform Pillars of Diabetes Prevention

A personal health coach

  • Each participant has their own dedicated health coach.

  • Coaches provide ongoing support by phone, instant messaging or in-app messaging.

Easy tracking

  • Each participant receives a wrist-worn digital activity tracker and a wireless bathroom scale to track their exercise and weight.

  • All the data measured by the bathroom scale and the digital activity tracker is automatically transmitted to the health coach. There is no need to keep a written journal of any kind.

  • Each participant shares their food choices with their Health Coach with an in-App photo journal.

Clinically-proven 12-month curriculum

  • Weekly learning sessions educate participants about diet, exercise, stress management and more.

  • Transform is a US Centers for Disease Control and Prevention (CDC) fully recognized Diabetes Prevention Program (DPP).

How Transform works:

Before the Program Starts

  • You will receive your wireless bathroom scale and digital activity tracker by mail.

  • You will download the Transform app and begin exploring its features and connect your activity tracker and scale.

  • You will meet your new health coach via email.

During the program

  • You will receive weekly learning sessions that will guide you through building a healthy lifestyle.

  • You will track your weight, physical activity and food effortlessly with the provided digital tracking tools.

  • You will have regular phone calls and messaging with your health coach to discuss your progress and support your goals.
